How do I get my Win/Loss Statement?

You can request your Win/Loss Statement in one of the following ways:

In Person (Fastest Option)
Visit the Players Club at either Odawa Casino location with a valid photo ID. Your Win/Loss Statement can be provided instantly during your visit.

By Mail or Email
If you’re unable to visit in person, you may request a Win/Loss Statement by calling the Players Club at 877-442-6464 ext. 8801 to request a Win/Loss Statement form. You may choose to have the form emailed or mailed to you.

To complete your request, you’ll need to:

Fill out the request form
Include a photocopy of a valid photo ID

Once received, your Win/Loss Statement will be sent back to you using your preferred delivery method. Please allow a few days for processing when requesting by mail or email.
